Healthcare Plans & Administering Medication Forms
It is important that we are kept regularly up to date with your child's healthcare needs. If your child has a health condition please tell us by completing an Individual Healthcare Plan.
If your child requires medication whilst in school, please also complete a Parental Agreement for Setting to Administer Medicine Form.
Please bring your child's medication to the school reception to be signed in. The school can only accept medication in its original packaging indicating the patient name and dosage that has been prescribed by a Dr and when both of the above forms have been received.
Lunches
Dolce provide free hot lunches for Reception, Year 1 and 2, although parents may wish to provide their own. Key Stage 2 will be able to purchase these or provide their own.

Please complete the attached form to inform us of your child's end of day arrangements.

School Uniform
Pupils should come to school in their PE kit on the days that they have this subject.
All school uniform, excluding ties and swimming hats, can be purchased online from Different Class Schoolwear. Differentclassschoolwear.co.uk
Ties are available to purchase from your child's class teacher. Please send your child into school with £4.20, in a named, sealed envelope.
Name Labels
Please remember to label all items of your child's uniform with their name. We recommend Stikins name labels, which can be easily stuck into clothes and shoes and onto bags, lunch boxes, drink bottles and pencil cases.
The school will receive a small commission from your purchase, when you enter our school fundraising number 22413. www.stikins.co.uk
If you do not wish to purchase name labels, please mark your child's belongings clearly with a pen.
